NEW STUDENT REGISTRATION
In order to provide an optimum teaching and learning situation for all students new to Valley High School, the following registration procedure will be followed.
- Parent/guardian will pick-up registration packet and schedule an appointment for registration with the counseling office.
- Parent/guardian will provide the following documentation. Please bring all documentation with you to your registration appointment.
- Immunization record
- Birth Certificate
- High school transcript
- Withdrawal form from previous school
- Proof of in-district residency (i.e. utility bill, rent/lease, mortgage documentation)
- Guardians must provide legal court order documentation of custodial rights.
- Parent/guardian must complete the following forms:
- Student Data Form
- Health Questionnaire
- Home Language Questionnaire
- Housing Form
- BOCES Migrant Survey
- Internet Usage Form
- Laptop Agreement
- Free/Reduced Lunch Form (optional)
- If special education needs apply, the parent/guardian will provide a current IEP and sign a consent form for release of records. A special education transition/transfer staffing will be required before the student may attend classes.
- No student will be allowed to enroll until a counselor/administrator contacts the previous school to obtain information about enrollment, attendance, discipline, classes/grades, special education, and juvenile justice involvement. A minimum of one day will be required for completion of this check.
- A final admittance or denial decision will be made by administration once the above information has been obtained.
